The questionnaire is a feature that allows you to ask questions to your users. It can take the form of a survey, a vote, a diagnosis, an inquiry, or a form.

How to configure a questionnaire?
- To create your questionnaire, go to the
Consultations
tab in your dashboard.
- Click
Create
at the top right of your screen, then select the Questionnaire
template.

How to customize the questions?
- In the
Questions
tab, specify the start and end dates of your consultation. You can add a description to introduce your questionnaire.
- Create your questionnaire in the
Questionnaire
section.
- Create a question by clicking
Add a question
.
- Enter the label of your question, select the answer format. You can add a description to your question to provide details about the expected response.

Each question format allows you to collect different types of information:

This field allows you to add clarifications, a recommended response format, a link to a source, or a bullet-point list.
Click Add a description
below your question and format your information accordingly.
